Washington State Health Survey

Students in grades 10 and 12 will be taking the Washington State Health Survey on October 27th, 2025 during their Social Studies class. Additional information will be sent out on Parent Square. The Parent and Youth Information letter, and a copy of the survey questions are available below. The survey is anonymous and voluntary. An opt-out form can be found in the Parent Square letter or with your student's grade-level counselor.
